NURSING HOME RELOCATION

   

RISE provides nursing home relocation coordination services to individuals currently living in a nursing home and who desire to move to an independent setting in their community.

What is Home By Choice?

Home By Choice is a program that is funded by the Texas Department of Aging and Disability Services (DADS). It helps people in nursing homes move back to the community and set up the services they want to support them.

Do you want to live at home?

Do you live in a nursing home and want to move back home? Do you want to live in your own apartment, with a roommate, or with your family? People living in nursing homes have the right to choose where they live.

How do I qualify for Home By Choice?

• You are Medicaid eligible
• You live in a nursing facility
• You reside in Jefferson, Hardin, Orange, Jasper, Newton, Tyler, Polk, San Jacinto, or Sabine county.

What services are available through Home By Choice?

RISE, and the Home By Choice Program, through a service collaboration with the Texas Association of Centers for Independent Living (TACIL) and the Texas Department of Aging and Disability Services (DADS) provides assistance with the following services:

• Housing or moving back home
• Information on living in the community with suitable support
• Nursing services
• Personal attendants
• Therapies
• Medical equipment and supplies
• Minor home modifications
• Prescriptions
• Case Management

It can also help you apply for special grants to help with expenses such as rent and utility deposits, and household furnishings.

How much do Home by Choice services cost?

Home By Choice services are provided at no cost to you.
